The hourly wage for this role is $44.64 to $58.15. Compensation is based on experience.

POSITION SUMMARY

Reporting to the Nursing Director, the Registered Nurse will deliver culturally sensitive medical services to low-income residents of Los Angeles County, with particular attention to the LGBTQ+ community. The RN will ensure high-quality, cost-effective patient care, manage treatment plans, and coordinate with providers and clinical staff to achieve positive outcomes within efficient timeframes and resource use. Additionally, the nurse will collaborate with the Medical Director and Nursing Supervisor to design, implement, and evaluate workflow processes related to patient-centered medical home (PCMH) implementation across all APLA Health sites. This is primarily a clinical position.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Offer clinical support to medical providers before and after primary care visits to facilitate timely, quality care.
  • Coordinate with medical assistants to ensure timely specialty referrals and follow-up to maintain continuity of care.
  • Treat patients with sexually transmitted infections (STIs) promptly and in accordance with APLAHW clinical guidelines.
  • Administer vaccines per APLAHW clinical protocols.
  • Deliver and properly document health education for patients.
  • Engage in quality improvement activities as directed by the medical director, such as data analysis, outcome measurement, and reporting on initiative results.
  • Triage walk-in patients and those calling with acute medical concerns.
  • Assist front office staff in rescheduling or accommodating late-arriving patients by reviewing records and determining appropriate timing and location for care.
  • Guide clinic LVNs and MAs by supervising front and back office staff to maintain efficiency, professionalism, and patient courtesy. This may include providing backup support when needed.
  • Support the clinic’s transition to a patient-centered medical home (PCMH) in collaboration with the PCMH coordinator, medical director, chief operating officer, and clinical nurse manager.
  • Maintain adequate medical supplies and medications by monitoring inventory and placing orders as necessary. Ensure exam rooms are stocked and ready for patient visits.
  • Participate in chronic disease management for patients with special needs.
  • Communicate laboratory results to patients.
  • Assist patients in accessing other APLA services, such as benefits counseling, mental health, case management, and dental care.
  • Serve as the initial point of contact for the eCW patient portal, responding within scope and forwarding inquiries to the appropriate provider when needed.
  • Obtain prior authorizations for medications.
  • Contact insurance carriers to verify medication eligibility and requirements.
  • Request, track, and secure prior authorizations from insurance companies.
  • Communicate insurance changes or trends to the team.
  • Clearly document all communications and contacts with insurance companies, pharmacy benefit organizations, and pharmacies.

Other duties may be assigned as needed to meet business requirements.

REQUIREMENTS

Training and Experience: A valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing (BRN). Current Basic Cardiac Life Support (BCLS) and Cardiopulmonary Resuscitation (CPR) certification. Clinical experience as a registered nurse in an ambulatory health care setting, preferably at a federally qualified health center (FQHC). Experience working in patient-centered care teams is preferred.

Must possess problem-solving and organizational skills to ensure a productive work environment and goal achievement. Experience in chronic disease management, case management, utilization management, and quality improvement projects is preferred. Experience in an HIV medical practice is also preferred.

Knowledge of: Familiarity and experience with electronic health records, preferably Eclinical Works.

Ability to: Provide services in a non-judgmental manner and work effectively with diverse populations. Must be able to maintain accurate records, follow clinical guidelines and protocols, work efficiently with a high degree of accuracy, solve problems independently, and work flexible hours to complete tasks and meet client needs. Flexibility in responding to unanticipated client needs is required.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS

This is primarily an office-based role that may occasionally involve bending, reaching, stooping, lifting, and moving office materials weighing up to 25 pounds. Daily use of a personal computer is required, including entering, viewing, and revising text and graphics on screen and on paper.

SPECIAL REQUIREMENTS

Must possess a valid California driver’s license, proof of automobile liability insurance, and have access to a personal vehicle for work-related purposes. COVID-19 vaccination is required; APLA Health will consider accommodations for disability- and religious-based reasons.
