The hourly wage for this role is $34.99 to $44.29. Compensation is based on experience.
POSITION SUMMARY:
Reporting directly to the Nursing Supervisor, the LVN III will deliver nursing services within the authorized scope of practice and in compliance with established care standards, policies, and protocols. The LVN III will also perform care coordination and health education tasks to involve patients in establishing goals and supporting self-management for improved health outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Care Coordination:
- Assist the care team in organizing patient care, which includes completing necessary documentation and facilitating patient access to suitable medical and specialty providers.
- Evaluate individual health requirements and supply educational materials to inform patients and their families about health topics for managing their conditions.
- Support patients with referrals, confirm appointment scheduling for referred patients, and manage the referral process to completion.
- Perform proactive patient outreach and schedule required follow-up appointments.
- Connect patients with community-based organizations and local services to address social determinants of health.
- Act as a specialist in community resources.
- Assist with medication refill requests.
- Collaborate with patients managing chronic conditions, focusing on patient priorities, education, goal setting, and self-management coaching.
- Deliver medical information and education to patients to aid in self-management of their health, adhering to established protocols.
- Conduct follow-up with patients between visits to monitor their progress.
Clinical:
- Execute patient care tasks within the scope of competency as directed by healthcare providers, following established protocols, policies, and procedures.
- Evaluate and monitor patient conditions, notifying healthcare providers for further direction.
- Transmit medication refill authorizations to pharmacies as instructed by healthcare providers per established protocols.
- Implement principles of aseptic technique and infection control as outlined in the Infection Control Manual.
- Record relevant patient information, assessments, and nursing procedures in accordance with established protocols.
- Properly administer oral, intramuscular, and subcutaneous medications.
- Perform venipuncture skillfully while adhering to all OSHA PPE safety standards.
- Conduct HIV and STD screening tests as needed or requested.
- Refer individuals with positive HIV or STD test results to Partner Counseling Services.
- Refer individuals with positive STD test results for immediate treatment.
- Link individuals with positive HIV test results to medical care and ensure follow-up to confirm successful linkage.
- Connect individuals with negative HIV or STD test results who are at high risk to appropriate prevention programs, substance use treatment, or mental health programs.
- Ensure referrals and linkages to substance use treatment and mental health support programs are up to date.
- Maintain HIV and STD records to ensure all documentation required by funders is properly filed.
- Overtime work may be required for essential shift coverage, including evenings and weekends, on an as-needed basis.
- May be required to work at various APLA Health sites as necessary.
OTHER DUTIES MAY BE ASSIGNED TO MEET BUSINESS NEEDS.
REQUIREMENTS:
Training and Experience:
- High school diploma or GED equivalent.
- Current California LVN license.
- Minimum of two years\' experience as a licensed vocational nurse in an Internal Medicine or Family Medicine practice is preferred.
- Current certification as an HIV Counselor or obtain certification within ninety days of employment.
- Experience working in a nonprofit environment is preferred.
- Skill and knowledge to maintain current LVN license/certification.
- Case management experience is preferred.
Knowledge of:
- Knowledge and experience working with target populations, including individuals who use IV drugs and sexual minorities, including transgender persons, is preferred.
- HIPAA certification and knowledge of informed consent.
- Knowledge of medical terminology.
- Knowledge of all safety requirements when handling bio-hazardous specimens or contaminated instruments.
- Knowledge of MSDS and emergency treatments.
- Knowledge of referral processing.
Ability to:
- Ability to use computerized patient care systems.
- Ability to perform accurate quality control and patient sample testing for waived tests.
- Work independently with minimal direction.
- Work effectively with diverse staff, volunteers, and professionals.
- Work comfortably and effectively with a diverse group of individuals across ethnicity, class, gender, and sexual orientation, who may hold a wide range of opinions regarding prevention needs.

- Maintain patient confidentiality.
- Ensure that HIV Counseling and Testing services are: 1) nonjudgmental, 2) harm reduction focused, 3) sex positive, and 4) LGBT positive.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
This is primarily an office-based role that involves only occasional bending, reaching, stooping, and lifting or moving office materials weighing 25 pounds or less. Daily use of a personal computer is required, involving entering, viewing, and editing text and graphics both on the computer and on paper.
SPECIAL REQUIREMENTS:
Must possess a valid California driver’s license; provide proof of automobile liability insurance; and have access to a personal vehicle for work-related purposes. Some weekend and evening work may be necessary. Local travel within Los Angeles County is required, with some out-of-town and national travel potentially required.
